Unusual evolution of acquired infundibular stenosis in patients with ventricular septal defect. Clinical and morphologic observations.

چکیده
Hemodynamic and cardiac morphologic observations were made on two patients with ventricular septal defect who showed unusual evolution of obstruction to right ventricular outflow. Both patients developed progressive infundibular stenosis, one in the presence of a spontaneously closing ventricular septal defect and the other after operative closure of the defect. Light and electron microscopic examination of resected infundibular muscle revealed hypertrophy and abnormal shapes and arrangements of cardiac muscle cells. These abnormalities resembled those found in left ventricular outflow tract muscle of patients with idiopathic hypertrophic subaortic stenosis. It is suggested that the pattems of distribution and growth of these abnormal cells in the right ventricular outflow tract may lead to localized hypertrophy and to development of progressive infundibular obstruction in some patients with ventricular septal defect.

Natural history of ventricular septal defect with right-sided aortic arch.
The natural history of 14 patients with ventricular septal defect and right-sided aortic arch is reviewed. Patients are divided into two groups. Group A consisted of 10 patients, all of whom developed progressive infundibular stenosis; group B consisted of 4 patients, none of whom developed infundibular stenosis. The clinical features, x-rays, electrocardiograms, haemodynamic data, and angiocar...

متن کاملA Rare Case of Double-Chambered Right Ventricle Associated with Ventricular Septal Defect and Congenital Absence of the Pulmonary Valve
Double-chambered right ventricle (DCRV) is a rare congenital heart disorder involving 2 different right ventricle (RV) pressure compartments that is often associated with ventricular septal defect (VSD). Usually, the obstruction is caused by an anomalous muscle bundle crossing the RV from the interventricular septum to the RV free wall. We are reporting a case of double-chambered right ventricl...
متن کاملCongenitally corrected transposition of great vessels with infundibular pulmonary stenosis with ventricular septal defect, presenting with atypical atrioventricular nodal reentrant tachycardia: a rare association
Congenitally corrected transposition of great vessels (CCTGV) is a rare congenital heart disease (CHD) accounting for <1% of CHDs. CCTGV with infundibular pulmonary stenosis (PS) with ventricular septal defect (VSD) is part of Fallot's physiology. It is known to be associated with bradyarrhythmias like atrioventricular (AV) blocks, and acquired complete AV block occurs at a rate of 2% per year....
متن کاملPulmonic Stenosis with Intact Ventricular Septum
Pulmonic stenosis unassociated with a ventricular septal defect of significant size is often a complex abnormality. The stenosis may be valvular, infundibular, or both, and may be associated with an atrial septal defect or even with a small ventricular septal defect. Preoperative studies cannot accurately predict these variations. The surgical approach selected for such patients should permit t...
